| TEAM $500 | Foursome Entry. Awards banquet with dinner, golf gift bag and team pictures. |
| INDIVIDUAL $125 | Individual Entry. Awards banquet with dinner, golf gift bag and picture. |
| MULLIGAN RAFFLE $25 | Maximum four mulligan’s per team. |
