| TEAM $500 | Foursome Entry. Awards banquet with dinner, golf gift bag and team pictures. ($320 is tax deductible) |
| INDIVIDUAL $125 | Individual Entry. Awards banquet with dinner, golf gift bag and picture.($80 is tax deductible) |
| MULLIGAN RAFFLE $25 | Maximum four mulligan’s per team. |
